SharePoint 2013 Form based authentication not working

Hello,

I tried to setup Windows authentication and Form based authentication in SharePoint 2013

Windows authentication is working fine. but when I try to login with Form based authentication access is denied.

I have added user in membership database (aspnetdb) from IIS -> .net users -> add users. Also used same user as site collection administrator.

Any suggestions would be highly appreciated.
