Content db vs SQL db

Hi

1. Our content db (200GB+) is larger than its wss SQL db (130GB). 

How is this possible and how can the last modified date on the SQL db be months ago if the content is changing daily?

It seems like the SQL db hasn't grown in years, but content has grown significantly. SharePoint admin console points to the the wss db on our SQL server.

2. The performance of the above site is extremely slow (working on it..., times out, workflows often fail). Other smaller sites with different content dbs and the same sql db have no issues. The SQL server shows no resource issues. Network performance issues have been excluded.

Would it be the size of the db causing performance issues? 

There has never been any maintenance of any kind. Server reboots made no difference.

Please help :'(
